5K RUN/WALK LIABILITY WAIVER & RELEASE FORM

PLEASE READ CAREFULLY — THIS IS A LEGAL DOCUMENT

In consideration of being permitted to participate in the Redwood Area Animal Shelter & Functional Fitness ‘Sunshine for Pets’ 5K Run/Walk (“Event”), I, the undersigned, acknowledge, appreciate, and agree to the following:

  1. Assumption of Risk
    I understand that participation in a running/walking event involves inherent risks, including but not limited to: falls, contact with other participants, weather conditions, traffic, road hazards, animal encounters, and physical exertion. I voluntarily assume all risks, known and unknown, even if arising from the negligence of the event organizers, sponsors, or volunteers.

  2. Release of Liability
    I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless the Redwood Area Animal Shelter, Functional Fitness, event sponsors, volunteers, staff, and all other event affiliates (“Releasees”) from any and all liability for injury, disability, death, or loss/damage to person or property arising from my participation in this Event, whether caused by negligence or otherwise, to the fullest extent permitted by law.

  3. Medical Authorization
    I consent to receive medical treatment deemed advisable in the event of injury, accident, or illness during my participation. I understand that I am solely responsible for any medical costs incurred as a result of such treatment.

  4. Fitness to Participate
    I certify that I am physically fit and sufficiently trained to safely participate in the Event, and that I have not been advised otherwise by a qualified medical professional.

  5. Photography/Media Release
    I grant permission to the Redwood Area Animal Shelter, Functional Fitness, and their agents to use photographs, video, and/or audio recordings of me for promotional purposes, without compensation.

  6. Minors
    If signing on behalf of a minor (under 18 years old), I certify that I am their parent or legal guardian and have the legal authority to sign this agreement. I consent to their participation and accept all terms of this waiver on their behalf.

I HAVE READ THIS WAIVER, FULLY UNDERSTAND ITS TERMS, AND COMPLETE IT FREELY AND VOLUNTARILY WITH THE INTENTION OF BEING LEGALLY BOUND.
